Biomail
Biomail allows you to set up a search in PubMed and choose how often you want the search results sent to you. You can choose up to ten different searches to review regularly.

Medscape
Medscape from WebMD offers weekly or daily email updates highlighting key medical news and features by specialty.  ACP Medicine and ACS Surgery What's New monthly email reports are also available.  Free but requires registration.

My NCBI
My NCBI allows you to save searches in PubMed and have the results delivered to you in your email. Users of this National Library of Medicine service can also employ Loansome Doc document delivery and limit to journals available in the Preston Medical Library.

PubCrawler
PubCrawler provides the capability of setting up searches using either the journal title or titles or a subject search using keyword or Medical Subject Headings, author, title, etc.
