How to Refer a Patient

 

We accept all referrals by fax to 865-305-6675

For scheduling a general genetics Pediatric/Prenatal/Adult Clinic Evaluation:

  1. Please fax one of our completed referral forms with the demographics and clinical information regarding the reason for referral (including office notes, labs, radiology reports, etc) and an enlarged front and back copy of the patient’s insurance card to 865-305-6675

  2. The faxed referral will be reviewed by the clinic coordinator, and if additional information is required, the referring physician’s office will be informed that additional information should be faxed to our office before the appointment can be scheduled.

  3. Once approved, the referring physician’s office will be notified via fax that the referral has been approved and the patient may call the UT Genetics Center scheduling line at 865-305-9030, option 3, to make an appointment.

  4. If we do not receive a call from the patient after 90 days, we will notify the referring physician’s office via fax that an appointment has not been scheduled. 

  5. Once the appointment is scheduled, a packet of information will be either emailed or mailed to the patient. The packet will include information about the appointment, directions to the office, and a questionnaire that should be completed and brought to our office the day of the appointment.

  1. Please fax one of our completed referral forms with the demographics and clinical information regarding the reason for referral (including office notes, labs, radiology reports, etc) and an enlarged front and back copy of the patient’s insurance card to 865-305-6675

  2. The faxed referral will be reviewed by the clinic coordinator, and if additional information is required, the referring physician’s office will be informed that additional information should be faxed to our office before the appointment can be scheduled.

  3. Once approved, our office will contact the patient to schedule an appointment. 

  4. If we are unable to make contact with the patient after 90 days, we will notify the referring physician’s office via fax that an appointment has not been scheduled. 

  5. Once the appointment is scheduled, a packet of information will be either emailed or mailed to the patient. The packet will include information about the appointment, directions to the office, and a questionnaire that should be completed and brought to our office the day of the appointment.
