Billing Information
Billing Policy
In-state (TN) Medicaid patients: The UT Genetics Center will file the patient’s insurance. Please provide a copy of the front and back of the patient’s insurance card with the specimen. Also, include the insured’s date of birth and relationship to the patient.
Out-of-state patient: The UT Genetics Center does not bill out-of-state insurance companies. In order to process the test, we must receive one of the following with the specimen: (1) Institutional billing information or (2) Check, Visa or MasterCard information from the patient.
